Sinakop ng mga Hapón ang Intramuros noong 1941-1942, kung saan ginawa nilang matibay na posisyon na may mga kanyon mula sa kanilang mga barko, na nagresulta sa Manila Massacre—pagpatay sa libu-libong sibilyan, kabilang ang mga hinostage sa San Agustin Church—at malawakang pagkasunog at pagbobomba ng mga Amerikano upang maibawi ito, na nag-iwan ng higit 16,000 Hapong patay at halos ganap na pagkasira ng distrito.

Sinimulan ang muling pagtatayo noong 1951 ng Intramuros Administration bilang national historical site, na nagbalik ng mga muerta (mga pader), simbahan tulad ng San Agustin (UNESCO site), at Fort Santiago sa pamamagitan ng restoration projects na pinamamahalaan ng pamahalaan hanggang ngayon.

Ngayong 2026, isa na itong sikat na tourist destination sa Manila na puno ng restored Spanish-era buildings, museums, at cultural events, na nagiging hub ng education, arts, at heritage tourism habang patuloy na pinoprotektahan bilang historical landmark.
